Dog Adoption Application

Adoption Application

We appreciate your interest in adopting an APPP program dog.  Please note that we only adopt our dogs out after completion of the entire ten-week program, therefore dogs will be released to their new homes twice a year: April and November. We typically start the application process in January and August for the two sessions; the process involves time to complete. To move forward, this application must be filled out in its entirety.

Please contact us if you need more information.

Fields marked with * are required

Please initial each statement:

You agree to pay the adoption fee for one of APPP's program dogs in the amount of $350. This fee  includes a $100 non-refundable deposit and is due midway through the program at the Meet-N-Greet.  Our fees help to cover APPP costs that include food, boarding, medical care, training, and supplies. All dogs will be spayed or neutered and vaccinated prior to going home. Adoption fees are nonrefundable.

Your adopted dog will have a great start on training and know lots of commands.  However, dogs will be dogs, and your new pet may display behaviors (get into the garbage, bark, jump on furniture, and so on) that require further training.  A transition period of adjustment is to be expected; allow your dog at least a couple months to adjust to your home and routines.  APPP will be available to provide further training resources and suggestions.

You agree to care for the dog to the best of your ability with the full intent of keeping the dog for the duration of it's life. If for any reason, you cannot keep the dog, you must contact APPP to decide jointly on an appropriate alternate home. APPP reserves the right to take the dog back if it is ever neglected or improperly cared for.

You understand that your dog will be microchipped before being adopted and registered with APPP's contact information.

You agree to provide suitable care for your dog whenever you will be away from home for any length of time (vacations, emergency, etc.).

You agree to provide shelter inside your home, good nutrition, and proper medical care for your dog.  You also agree that the dog will not run free without supervision, will not be chained outside, and will be properly supervised around children.  Your dog will not be allowed to ride loose in the back of a truck, will not be left in a hot vehicle, or be used for guarding purposes. Failure to comply with these terms will result in the dog being immediately returned to APPP without a refund.

You agree that all of your household family members will attend either the Meet-N-Greet or Graduation Day for introduction to their new dog.

You agree to show proof that you are at least 21 years of age, if requested by APPP.
